Reporting a parsing problem
Parsing problems are almost always document-shaped: whether the file is a .docx or a PDF, whether it was exported from Word or scanned, and how the questions and choices are laid out on the page. Include that detail and, if you can share it, the document itself — it is the fastest way to see what the parser saw.
